What it covers.

A Health and Welfare LPA can cover decisions such as where you live, day-to-day care and medical treatment. It can only be used when you lack capacity to make the decision yourself.

You can record preferences and instructions, including your views on life-sustaining treatment, which needs particular care in how the form is completed. We will explain what is involved.

It must be registered with the Office of the Public Guardian. Appropriate arrangements for the certificate provider will be discussed with you. We do not automatically act as certificate provider.
